reported title tracking

As far as I can see, when we report a title for any reason, such as having incorrect data, there is no way to track what we have already reported, or to know if something has already been reported for that reason.

It would be neat if we could have either a list of titles we have personally reported with a reason listed, or maybe some indication that a title has already been reported by someone else.

this would save a duplicate report from being created (either by a user because they forgot they already reported or because someone else has), especially if staff are busy and it takes a little time for them to resolve the reported issue.
